Practice Closure

After many years of serving patients, this practice is now permanently closed as of May 31st.

If you are a former patient and need assistance obtaining medical records or related documentation, please use the form provided and someone will respond as soon as possible.

If you are experiencing a psychiatric or medical emergency, call 911 or go to the nearest emergency room.

For immediate mental health crisis support, contact the 988 Suicide & Crisis Lifeline by dialing 988.

Our office will be closed December 24th, 2025 – February 4th, 2026.
We will reopen on, January 5, 2026.
